Payment for Services & Insurance

Meet & Greet

Unless you're a returning client, I require a brief meet & greet session so I can understand what you're looking for and whether I'm a good clinical fit. This can happen by phone, video (Google Meet), or in person — contact my office to set one up.


Health Insurance

I accept many common insurance plans, but coverage varies by employer and plan — please verify directly with your insurer whether I'm in-network for your specific plan. This is especially worth double-checking with Kaiser Permanente and some trade-sponsored health plans.


I do not accept or bill:

  • Medicare or Medicare secondary payer plans

  • Medicaid-type plans (e.g., Apple Health, Molina, Community Health Plan)

  • Automobile insurance claims

  • Employee Assistance Programs (EAP)

 

Paying Out of Pocket

My cash rate is listed in my Professional Disclosure (below). I accept credit cards and checks; cash payments must be made directly to me, not office staff.

Preparing for Your First Visit

After your meet & greet, download and complete the required forms below. You'll also need a photocopy of a government-issued photo ID and your insurance card (if using insurance).

 

  • Telehealth clients: return forms by mail or via secure email (below). A separate Telehealth Agreement is also required before your first session.

  • In-person clients: bring forms with you, or send them ahead via secure email. Please don't arrive early — we keep waiting-room time to a minimum.

  1. New Client Intake — complete and sign

  2. Client Billing Information — only if using insurance

  3. Professional Disclosure — initial each page and sign

  4. Telehealth Services Agreement — only if doing therapy online or by phone

  5. Release of Confidential Information — only if you want me to share information with others

  6. Medicare Opt-Out Form — required if you're Medicare-eligible, since my practice doesn't participate in Medicare

Scope of Practice

As a master's-level clinician, I can't prescribe medication. I work with adults aged 21 and up. A few other things fall outside my practice, so you can plan accordingly:

 

  • One-time ("one & done") sessions solely to complete paperwork or forms

  • Letters for comfort/emotional support animals

  • Court-ordered legal assessments

  • Court-ordered anger management or domestic violence classes

  • Work with sex offenders or those who have acted out with children

If you're not sure whether your situation is a fit, reach out — I'm happy to point you toward the right resource if I'm not it.
