Insurance and Payment for Services
To use your health insurance for my services, you first need to first verify with your insurance company or my office staff whether I accept your insurance. I currently accept many of the most common insurance plans.
If you are not using insurance, my cash rate is listed in my professional disclosure. My office can process credit card payments, check, or cash.
Your First Visit
Once an appointment is scheduled, download, complete, and bring the appropriate forms to your first office visit. As an alternative, you can arrive early to the first session to receive and complete the forms. Allow 30-45 minutes for this review.
When you arrive for your first appointment, my office needs to make a photocopy of your current insurance card and a government issued photo ID so be sure to bring them to your first office visit.

The following forms need to be completed, and or signed (unless you have questions), before your first visit.
For sessions conducted via Telehealth, I can sent these via a secure email and, for now, if you agree, simply respond to that email by saying I agree to abide by the terms of the Agreement for Teletherapy.
If you have questions, we can discuss during our Teletherapy session. The secure email address is listed under the "Contact" tab.
For in person sessions, you can download the forms, bring them to your first appointment. You also have the option of arriving early to my office early (30-40 minutes) to complete the forms manually.
Due to legal and privacy considerations, do not return these forms via e-mail, text, other than DocuSign, or by other prearranged agreement.
